Contrary to popular belief, the neck does not work in isolation. The ribs, thoracic spine, diaphragm, and small stabilizing muscles of the neck all work together during breathing and everyday movement. Research has shown that people with chronic neck pain often demonstrate respiratory dysfunction, reduced chest mobility, and links between thoracic movement, respiratory function, and neck disability (Wirth et al., 2014; Han et al., 2020).


When breathing becomes shallow or “upper chest dominant,” the body tends to rely more heavily on accessory breathing muscles such as the scalenes, sternocleidomastoid, and upper trapezius. These are the same muscles that often become tight, overworked, and tender in people with neck pain. Over time, poor breathing mechanics can add unnecessary tension to an already irritated cervical spine and make recovery more difficult.


The Role of Chiropractic Care


Restricted rib mobility can make this worse. If the rib cage and thoracic spine do not expand and rotate well, the neck may compensate during breathing, reaching, lifting, and posture changes. Studies have found that chronic neck pain is associated with reduced thoracic mobility during forced respiration and that these changes may contribute to impaired respiratory strength (Han et al., 2020). Other research has also shown that thoracic joint mobilization and self-stretching exercises can improve pulmonary function in patients with chronic neck pain (Kim et al., 2015).
